Ren Ci Terms & Agreement

1. I declare that the information given in this application form is true and complete. I understand that if I have given any misleading information on this form or made any omissions on purpose pertaining to myself, this will be sufficient grounds for Ren Ci Hospital for not accepting my application as a volunteer with the organisation.
2. I also understand that my involvement in the services of Ren Ci Hospital is solely on a voluntary basis. I recognise that there may be certain inherent risks in the voluntary activities and I will at all times indemnify Ren Ci Hospital and its staff on a full indemnity basis and hold Ren Ci and its staff harmless and against all actions, losses, mishap, injury, loss of life, claims, demands of whatever nature, proceedings, damages, costs and any other liability arising in any way from my participation in the activity or any breach of my undertakings hereof.
3. I will notify the Volunteer Coordinator whenever a conflict of interest arises.
4. As an individual volunteer, I have read and agreed to adhere to the (Ren Ci Programme Guidelines for Volunteers).
5. I understand and consent to be contacted by Ren Ci Hospital via Mail, E-mail and Phone Calls/Text Messages for volunteer-related matters, and to facilitate my volunteer service.
